RESIDENTIAL SOCIAL WORKER – Cordilleras Suites


STATEMENT OF PURPOSE

Telecare will deliver and manage excellent services and systems of care for persons with serious mental illness.

Employment Status: Full-TimeTEMPORARY

Shift: AM as needed

Days: Monday - Fridayas needed


POSITION OBJECTIVE

The Residential Social Worker provides or ensures comprehensive case management services to assigned Residents. Additionally, the Residential Social Worker is responsible for the on-going clinical and recovery assessments of all assigned Residents; including crisis interventions and other therapeutic, recovery-based clinical interventions, as needed.


QUALIFICATIONS

Bachelor’s Degree in Social Work, Psychology/Counseling or other related field is required; Masters Degree strongly preferred. Two (2) years work experience in psychiatric setting or other mental health setting (outpatient or inpatient) is preferred; however, experience with case management, social rehabilitation, crisis intervention and other related fields may be considered. Specific work experience may be waived for Master’s Degree. Applicant must possess a working knowledge of (1) the process of mental illness, (2) the Diagnostic and Statistical Manual of Mental Disorders (DSM) and (3) recovery philosophy and psychiatric rehabilitation principles. Specialized knowledge or experience with Dialectical Behavior Therapy (DBT) or cognitive behavioral therapy principles, in general, and borderline personality disorder preferred. Specialized knowledge/experience around dual diagnosis is extremely helpful. Necessary skills include the ability to work effectively as a member of a multidisciplinary team, a thorough understanding of client rights, an understanding of the conservatorship (LPS) process and knowledge of psychiatric terminology and basic medical terminology. First Aide and CPR certified or obtain certification within two (2) weeks of hire. Possess a valid California Driver 's License, be insurable and have access to a car. Applicant must be finger printed, sign under penalty of perjury that there has been no conviction of any crime, other than minor traffic offenses, and receive clearance from the Department of Justice/FBI. Computer literate (knowledge of network systems, internet, Windows, as well as Word, Excel, and case management documentation programs). Have the ability to read, write and speak English fluently.

QUALITY AND QUANTITY OF WORK
  • Provides safe, effective, efficient, culturally competent and Resident-centered implementation of direct care in accordance with established policies, procedures, and standards of care.
  • Assures that Residents know and understand their legal status and rights. Tracks legal status on all Residents.
  • Establishes and maintains a therapeutic relationship with Residents.
  • Serves as a DBT skills coach to DBT program Residents.
  • Facilitates and/or co-facilitates weekly Resident groups to promote the social, physical, emotional and psychological well-being of Residents.
  • Assumes comprehensive case management responsibilities of assigned Residents (15-17).
  • Meets 1:1 on a regular basis with assigned Residents to assess progress, review Resident goals, provide therapeutic support, and continue to develop therapeutic relationship.
  • Performs psychosocial interviews with Residents of the program.
  • Schedules appointments, communicates with conservators and treatment team personnel, coordinates consumer services etc.
  • Works collaboratively with County and Community agencies to meet Residents’ needs.
  • Utilizes the Behavior Protocol /Chain Analysis tools, as well as other clinical tools, with Residents as therapeutic interventions.
  • Orders, logs and distributes medication.
  • Provides daily living assistance including promoting and assisting Residents in developing self-help skills.
  • Performs service coordination functions of assessing, planning, linking, monitoring, and advocating on assigned Residents.
  • Transports Residents being served in company vehicles or personal vehicle, as needed.
  • Responds to crisis situations.
  • Completes daily “CAMINAR” computer Treatment/Care plans and progress notes, including documentation of all aspects of case management, rehabilitation groups and crisis interventions in compliance with the San Mateo County Mental Health Services Plan.
  • Audits the resident charts to ensure compliance with Community Care Licensing (CCL) and (CARF) requirements.
  • Supervises and audits the medication management to ensure compliance with CCL and CARF requirements.
  • Completes Telecare internal Incident Reports and maintains individual Resident Incident Logs in compliance with established policies and procedures.
  • Notifies appropriate agencies of any known or observed incidents of Resident abuse.
  • Completes other duties as assigned.


TEAM MEMBER PARTICIPATION

  • Participates as a full team member and provides input via reporting observations, concerns, and asking appropriate questions.
  • Demonstrates knowledge of Professional Assault Response Training (PART) techniques.
  • Participates in the development, implementation and update of Resident Crisis and Service Plans.
  • Actively participates in multidisciplinary team meetings and service planning meetings as necessary.
  • Works directly with the Residential Director in drafting Resident behavior contracts and eviction notices.
  • Assists the Residential Director in the interview/assessment process of referrals.
  • Collaborates with Residential Director and other Team members regarding Residents’ readiness and appropriateness for discharge.
  • Facilitates discharge planning, including making appropriate referrals for placements, case management and aftercare services; completes required discharge paperwork.
  • Provides 24-hour on-call “after hours” coverage on a rotating basis.


ASSESSMENT

  • Observes, records, and reports Resident psychiatric behavior, social and vocational progress, ADL, substance abuse, per Telecare, County and State guidelines.
  • Updates the CCL Appraisal Needs/Services Plan annually, or as needed.
  • Updates any other Telecare, County, State, or CCL assessment forms as needed.
