Cordilleras is a licensed 68-bed mental health rehabilitation center (MHRC) and a licensed 49-bed adult residential facility (ARF) called The Suites. Cordilleras primarily serves San Mateo County residents, 18-59, with long histories of mental illness and multiple episodes of acute psychiatric hospitalization.

Employment Status: Full-Time .8

Shift: AM as needed

Days: Monday - Friday as needed

MISSION STATEMENT

Telecare will deliver and manage excellent services and systems of care for persons with serious mental illness.


POSITION OBJECTIVE

The Personal Service Coordinator II Provides or ensures the provision of services needed and wanted by members served in the most appropriate environment of members ' choice on a regular and emergency basis.


QUALIFICATIONS

One (1) year of experience in a psychiatric setting is required. B.S./B.A. or M.S./M.A. in Social Work, Nursing, Occupational, Recreational, Music, Dance, Drama or Art Therapy or Vocational Counseling or related fields is preferred. Direct vocational/substance abuse experience in the community preferred. Must have valid California driver 's license and access to a car and insurability, must have knowledge and ability to use public transportation. Knowledge of process of mental illness required. Applicant must be finger printed, sign under penalty of perjury that there has been no conviction of any crime, other than minor traffic offenses, and receive clearance from the Department of Justice. Specialized knowledge/experience in vocational, housing, or dual diagnosis services preferred. Bilingual abilities preferred.

  • Provides safe, effective, efficient, culturally competent and consumer centered implementation of direct care in accordance with established policies, procedures, and standards of care.
  • Establishes and maintains and co-facilitates a therapeutic relationship with members.
  • Performs psychosocial interviews with members of program.
  • Performs service coordination functions of assessing, planning, linking, monitoring, and advocating on assigned members.
  • Participates as active member of Team multidisciplinary staff.
  • Performs extensive in vivo services as requested/required to members ' homes, hospital emergency room(s), and other settings. Participates in planning and implementation of consumer treatment plans with emphasis on substance abuse and vocational needs.
  • Ensures documentation of all services in accordance with standards.
  • Performs liaison duties with assigned agency.
  • Collaborates in coordination of consumer services.
  • Transports persons being served, as needed.
  • Completes other duties as assigned.
  • Makes every effort to empower members to improve quality of everyday lives.
  • Participates in the development of the Individual Service Plan.
  • Coordinates with member for all needs to live successfully in the community.
  • Assumes professional responsibility for consumers well-being.
  • Responds to crisis situations.
  • Assure that members know and understand their legal status and rights. Tracks legal status on all members.


TEAM MEMBER PARTICIPATION

  • Participates as a full team member and provides input via reporting observations, concerns, and asking appropriate questions.
  • Reviews and implements member treatment/service plans as directed.
  • Actively participates in multidisciplinary team meetings and service planning meetings as necessary.
  • Provides 24-hour on-call (beeper) coverage on a rotating basis.
  • Accepts risks inherent in working in the community.


ASSESSMENT

  • Observes, records, and reports member social, ADL, substance abuse, vocational and psychiatric behavior per rehab option guidelines.
  • Completes the service plan, including assessing the members educational, substance abuse and vocational needs.

  • Actively participating in maintaining the inventory and ordering of equipment and supplies.
  • Monitoring the upkeep and safety of equipment and supplies.

    • Constant sitting and standing 8 hours per day.
    • Frequent bending, stooping, twisting, reaching, turning, and squatting.
    • Occasional pushing up to 30 pounds for 170 feet (length of hall)
    • Occasional pulling up to 25 pounds, possible wheel chairs.
    • Infrequent lifting 1-25 pounds from floor to overhead.
    • Occasional lifting 1-25 pounds from floor to waist.
    • Infrequent lifting 25-50 pounds from floor to waist.
    • Occasional lifting carrying of objects and materials weighing 65 to 70 pounds short distances of 15 to 30 feet.
    • Rarely carrying of patients 80 pounds to 300+ pounds. Other employees normally assist. Lift and carry is normally not aided by mechanical device.
    • Infrequent overhead reaching.
    • Constant small motor and hand skills necessary to perform writing tasks and other simple grasping operations.
    • Occasional fine manipulation needed.
    • Occasional power grip and hand twisting required in both hands for clutching and grasping; to seize or hold firmly.
