Telecare is proud to partner with Alameda County Behavioral Health Services in the development of a “behavioral-health-enriched” unit for 20 unhoused adults with behavioral health challenges and COVID-19. The Telecare Comfort Q Crisis Residential Treatment (CRT) will provide behavioral health, daily psychiatric services, and 24/7 nursing support. The Oakland, CA based site will be staffed with an interdisciplinary team consisting of Licensed (or License Eligible) Clinicians, RNs, LVNs, Residential Counselors, and Peers.
The program will be operational until the end of 2020 with potential for an extension. Telecare will make every effort to identify permanent opportunities for Comfort Q Crisis employees.
Employment Status: Multiple Openings - Full-Time, Part-Time (Short-Hour), or On-Call
The Peer Health Navigator engages and connects members served to mental health, primary care, substance use, and specialty health care services; helps them develop personal health and wellness goals; educates them on how to advocate for themselves and effectively communicate their needs; and supports them in overcoming barriers and building resilience. The Peer Health Navigator supports members served in maintaining a follow-up care plan and developing other self-management skills through a “Modeling – navigator performs task; member served observes, Coaching – member served performs task; navigator coaches, Fading – member served self-directs and manages own healthcare; navigator assist as needed” process. The Peer Health Navigator simultaneously serves as a role model of resilience, recovery, and self-responsibility for their own mental and physical health. They continually promote and reinforce Telecare’s Recovery Culture as defined by the Telecare Recovery Centered Clinical System (RCCS) in which the member’s point of view and preferences are recognized, understood, respected and integrated into services and self-help programming. Additionally, Peer Health Navigators will respond to critical situations with high-level engagement and de-escalation skills which support a least restrictive environment for members served experiencing an emergency related to a mental health/addiction challenge.
• High School Diploma or GED required
• Certified Peer Specialist and/or completed Peer Support Training
• Completion of a Health Navigator training program or completion within one (1) year of hire
• Two (2) years’ experience or part-time equivalent supporting individuals with behavioral health challenges
• Experience supporting individuals with physical health challenges to navigate the physical health care system
• Experience as a beneficiary of the public Behavioral Health system of care
• Successful completion of Peer Support Training or Peer Employment Training.
• Must be at least 18 years of age
• Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment
• All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. If the position requires driving, valid driver’s license, a motor vehicle clearance, and proof of auto insurance is required at time of employment and must be maintained throughout employment. • Draws upon own personal recovery experience with mental health/addiction challenges as a basis for engagement as a peer with the members served and to validate members’ own lived experiences
• Assists members served with voicing their strengths, interests, and goals throughout their time in services including: goal plans, crisis plans and other methods of expressing individual preferences
• Utilizes a comprehensive resource guide and educates members about local community supports and resources
• Supports members served in developing strategies to reduce self-stigma
• Engages and communicates with a wide range of members served especially those who are struggling to be active in their own recovery
• Assists participants in all needs including Activities of Daily Living (ADLs)
• Actively participates in multidisciplinary team meetings
• Participates in verbal de-escalation and physical interventions in emergent situations and is willing and able to assist other staff as needed to maintain a safe, healing environment for all
• Travels locally to provide outreach to members served in the community (program specific)
• Must assist with restraint of members served in the event of assaultive behavior and pass assault crisis/crisis prevention training

• Knowledge of recovery focused, strengths based work in mental health/addiction recovery
• Willing to seek to understand members’ unique circumstances, personal preferences, and goals and incorporate them into daily work as well as any needed crisis response to help them regain a sense of control
• Have a strong understanding of recovery and resilience, the value of partnerships with members and member choice, and the balance between protection from harm and personal dignity
• Strong knowledge of relevant community resources and methods for accessing them
• Possess strong communication, writing skills, and typing/computer skills
• Experience working with adults facing mental health/addiction challenges
• Interest or experience in working with people who are difficult to engage and maintain in traditional mental health programs
• Ability to communicate and work effectively with members served, family members, and staff from various backgrounds
The physical demands here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
The employee is occasionally required to stand, twist, squat, kneel and lift and carry items weighing 25 pounds or less as well as to frequently sit, walk, reach, bend and do simple and power grasping. The position requires manual deviation, repetition and dexterity.
