  • Our clients suffer from severe schizophrenia, schizoaffective, bipolar I and other diagnoses. Due to their severe mental illness, it is likely that the clients you serve will have homelessness, justice involved history, drug and alcohol addiction, comorbidities, and more. We use a person-centered approach, believing that recovery is possible for all people, and work every day to coordinate excellent care for the members on their journey to recovery. We believe recovery looks different for everyone and we strive to individualize services to restore dignity. We serve our clients from a posture of respect, kindness, and non-judgment.
  • Each Mental Health Specialist sees 3-5 clients per day and must account for 27.5 hours per week as client productivity. We drive out into the community to meet with our clients where they live and provide services.

What qualifications must you have to qualify and be successful in this role?

  • Master's degree in Social Sciences and two (2) years of experience providing direct services in Mental Health; OR Bachelor's degree in Social Sciences and four (4) years of experience providing direct services in Mental Health; OR an associate degree and six (6) years of experience providing direct services in Mental Health
  • You must have your own vehicle, willing to use it for work and willing to transport members in your car. You will be reimbursed for all your miles driven for the job.
  • You must have a driver 's license that is valid and current. Must have an acceptable Motor Vehicle Report Background Check and meet the driving standards of Telecare 's Fleet Safety Manual
  • You must have valid and current car insurance with your name listed as a driver.
  • Experience in San Diego County EHR Cerner is a big plus!
  • You must want to work Monday - Friday, 8:00am - 5:00pm.
  • Must be willing to rotate a Crisis After Hours cell phone with the rest of the staff (typically employees take the crisis cell phone home, at minimum, 4 times per year)

What will the job entail?

The Mental Health Rehabilitation Specialist provides members with community support and case management services using Psychiatric Rehabilitation principles, Recovery Philosophy, and Evidence-Based practices. This position provides education and support to members served and their families. You will coordinate service needs and collaborates with other services and agencies such as the Conservator 's Office, Public Defender 's Office, Child and adult Protective Services, Probation Department, and Inpatient Services, Long Term Care Services, Residential Settings and County Mental Health Services. The Mental Health Rehabilitation Specialist completes assessments, provides mental health rehabilitation services, case management, crisis intervention, integrated whole person care, and trauma informed care. Each day you will be required to provide 5.5 hours of member/client care. Most of those services will occur in the community. The other 2.5 hours each day are reserved for Team Morning Meeting, administrative needs, training, coaching with your supervisor, sometimes driving to appointments, completing other assigned tasks.
