Case Manager, Crisis Stabilization Unit

Telecare 's mission is to deliver excellent and effective behavioral health services that engage individuals with complex needs in recovering their health, hopes, and dreams. Telecare has over 170 programs and 4,500 employees across the country, and we are constantly growing and opening new programs.

The Windsor Center is a short-stay crisis stabilization unit (CSU) that offers 24/7 services to adults and youth. The Windsor Center aims to increase access to crisis services, reduce inpatient hospitalization, reduce the amount of time that law enforcement is involved in a mental health crisis, and strengthen the existing outpatient behavioral health services.

What are the qualifications? (Must Have)

  • Master's Degree in Social Services
  • 1 Year Experience in Mental Health Services
  • This position requires driving members in the community, including driving members in your own vehicle.
  • A valid and current driver 's license and car insurance
  • An acceptable Motor Vehicle Report
  • Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment

What will the job entail?

  • Collaborate with members served to create Recovery Plan goals
  • Ensure all services described in the Recovery Plan are provided
  • Work effectively as part of a multidisciplinary team to provide safe, effective and efficient implementation of direct care in accordance with established policies, procedures and standards of care
  • Collaborate with members served to create treatment plan goals, facilitate groups, interpret and implement service plans independently
  • Provide appropriate interventions in crisis situations
  • Collect required data and complete required documentation
  • Make appropriate recommendations for living environments, therapy, psychiatrist visits, case management and other services
  • Document and communicate interactions with members served
  • Provide support to team through direct assignment by supervisor

*Please note: Our clients suffer from severe schizophrenia, schizoaffective, bipolar I, and other diagnoses. Due to their severe mental illness, the clients you serve will likely have homelessness, justice-involved history, drug and alcohol addiction, comorbidities, and more. We use a person-centered approach, believing that recovery is possible for all people, and work every day to coordinate excellent care for the members on their journey to recovery.
