Telecare 's mission is to deliver excellent and effective behavioral health services that engage individuals with complex needs in recovering their health, hopes, and dreams. Telecare has over 170 programs and 4,500 employees across the country.

Home First is Orange County's Housing Full-Service Partnership (FSP) program focused on providing housing-supportive services and behavioral health services to individuals residing in Orange County. Our purpose is to support members in maintaining stable housing, including resolving housing challenges and providing case management and behavioral health-focused services.

Our staff create a supportive environment, including services that are based on individual needs, hopes, and dreams. Home First FSP members have access to a multidisciplinary team that includes peer specialists, case managers, a psychiatrist, a nurse practitioner, masters-level team leads, and clinicians who have experience in providing housing-supportive services and behavioral health-focused services.

Our program is based on the Assertive Community Treatment (ACT) model and Housing First Principles. This means that our goal is to help members become independent, productive, and integrate back into their community. We also connect members to the proper resources that can help them achieve their goals.

What are the qualifications? (Must Have)

  • High school diploma or GED + 2 years of mental health experience OR Associates Degree + 1 year of mental health experience OR Bachelor 's Degree in Social Sciences
  • You must have your own vehicle, willing to use it for work and willing to transport members in your car. You will be reimbursed for all your miles driven for the job.
  • You must have a driver 's license that is valid and current. Must have an acceptable Motor Vehicle Report Background Check and meet the driving standards of Telecare 's Fleet Safety Manual
  • You must have valid and current car insurance with your name listed as a driver.

What will the job entail?

The Case Manager provides members with community support and case management services using Psychiatric Rehabilitation principles, Recovery Philosophy, and Evidence-Based practices. This position provides education and support to members served and their families. You will coordinate service needs and collaborates with other services and agencies such as the Conservator 's Office, Public Defender 's Office, Child and adult Protective Services, Probation Department, and Inpatient Services, Long Term Care Services, Residential Settings and County Mental Health Services. The roles involves case management, crisis intervention, integrated whole person care, and trauma informed care. Each day you will be required to provide 5.5 hours of member/client care. Most of those services will occur in the community. The other 2.5 hours each day are reserved for Team Morning Meeting, administrative needs, training, coaching with your supervisor, sometimes driving to appointments, completing other assigned tasks.

*Please note: Our clients suffer from severe schizophrenia, schizoaffective, bipolar I, and other diagnoses. Due to their severe mental illness, the clients you serve will likely have homelessness, justice-involved history, drug and alcohol addiction, comorbidities, and more. We use a person-centered approach, believing that recovery is possible for all people, and work every day to coordinate excellent care for the members on their journey to recovery.
